COMPLETE ADDRESS
TELEPHONE NUMBER
BPI ACCOUNT NUMBER ( savings account only)
-If you have existing account, forward it immediately.
-If you don’t have account, open a new BPI savings account.
JAI or JOINT ACCOUNT INDICATOR
(2-digit no. located at the lower right hand corner
of your ATM card across your name)
